MDRD V's Cockcroft and Gault for drug dosing?

Dear all,

Sorry to bother you with this topic but we are currently up-dating our hospital formulary and I am reviewing the renal chapter of this publication.

One of my questions is how other hospitals have dealt with the issue of drug dosing in terms of the guidance now given between MDRD and the Cockcroft and Gault (C and G) equation which would influence prescribing across the hospital.

I am aware of the guidance now given in the literature and have researched this topic but need to confer with our consultants on how to move forward on this issue.

So, I was just wondering what advice is currently being followed in other hospitals in terms of drug dosage between MDRD and C and G? Has a distinction been made to exclude the different categories of patients for whom MDRD should not be used to predict dosage and has a specific list of medications for which MDRD should not be used been drawn up?

Any feed-back on this issue would be greatly appreciated in terms of what has been agreed for drug dosing in the hospital setting or in terms of advice from other authorities specifically pharmacy related groups from whom there seems to be a paucity of advice! 

Pointing me in the direction of some robust articles on this topic would also be greatly appreciated even though I have done a search on this topic.
