argatroban for HIT

We decided to use argatroban during the danaparoid shortage to anticoagulate our pts w/ HIT. I have found a couple references suggesting a bolus dose followed by an infusion. However, I have not been able to find a reference describing how to mix the bolus with a diluent/what concentration/rate of administration/over what time frame. Can anyone help??
