Home dialysis pharmacist involvement

We are just starting out with our home dialysis program. Does anyone have a dedicated pharmacist for this population? Also looking for some input regarding medication considerations for home hemo pts, more specifically re: what to do with the iv iron (given "in-centre" or self administered); how to withdraw the PO4 binders/which one first/how fast, or anything else re:home hemo you'd like to share.
