Documentation of Meds

Hi all,

My hospital is trying to move forward to writing a standard for medication charting in Haemodialysis. I was wondering if any other HD facilty has procedures/protocol on the charting of meds that they can share with us. We seem to have too many forms where pt's meds can be documented and I'm trying to streamline them. Can anyone share with me the forms that they use for documentation of inpt meds/ outpt meds/ meds given during dialysis. Our current documentation practice is a risk management issue and I'm worried one day a serious med error will be made especially with outpts that are also inpts. Are other HD facilities struggling with the same problem? Are there any facilities that use the same chart for both outpt and inpatients?

Any info, relevant documents or experience that you can share with me will be most apprepriated.
