Dosing of etoposide, carboplatin & ifosfamide in hemodialysis patients

A young male (in his 30's) with germ cell cancer. Previously treated with cisplatin and developed ototoxicity and neuropathy. Oncologist requesting dose of carboplatin and also asking when to have patient receive dialysis in relationship to chemotherapy. Patient also to receive etoposide and ifosfamide.
