FYI we had success in our program getting Mirapex covered for a hemodialysis patient whom was having a hard time with muscle spasms, restlessness and cramping on dialysis through Section 8 approval. Not an approved indication but may be something to think of if other tradional meds don't work for your patients. We did not use a standard Section 8 form and it took the doctor having to rewrite it back with more details (if he would have listened to me in the first place...) before the approval was given. Persistance is the key.
