venofer coverage change

to my surprise i've found out that starting some time next year venofer will require section 8 approval, this includes all patients receiving it now.

our site has been billing it as an extemporaneous prep (part 1, section 9 of formulary) without issues. in august the DQTC reviewed the drug and decided that it have section 8 status. this regulation will take place when the updates to the formulary come out.
