Section 8 in Ontario

I have had requests lately from various units about standardized forms for use in the Section 8, Individual Clinical Review program. Please see the shared information area of this wedbsite as I have uploaded the forms we currently use at our site. We have had success with many of these forms. We normally do not apply for Section 8 coverage for Replavite or Calcium Carbonate (relatively inexpensive form ost patietns) but in extenuating circumstances, I could see a case for application. We usually use Plavix, Renagel, Calcium acetate, Atarax (if used alot)Gabapentin, Avandia(requires HbA1c and max doses of glyburide tried first).
