IV iron and levocarnitine coverage

I know this isn't a new issue, but it is a new issue for us as Children's Hospital of Eastern Ontario. How many dialysis units are charging their patients for IV iron and/or levocarnitine, or have the patients bring in their own supply? Have you been successful in getting section 8 coverage for Venofer and levocarnitine? Any issues from insurance companies if patients have private drug coverage?
