PCP treatment

Does anyone have experience with dosing of Septra (trimethoprim/sulfamethoxazole) IV for treatment of pneumocystis carini pneumonia in a hemodialysis patient.
Dosing in RF seems unclear/contradictory for Septra amoung the references: Sandford/Micromedex/Renal Drug Book.
Or have you used alternate therapy ie. prednisone clindamycin/primaquine OR pentamidine?
Any advice/suggestion would be appreciated asap.
