Pediatric Iron Sucrose Dosing

I am currently reviewing our IV monograph for Iron Sucrose (Venofer). We currently have a formula to determine the total iron deficit in the pediatric patient:
IBW [kg] x (target Hg - actual Hg)[g/L] x 0.24 + depot iron [mg]
where depot iron = 15mg/kg if < 35kg or 500 mg if >/= 35kg
Do any other centres use this formula? If so, does anyone know where it came from and what the depot iron value represents?
What dosing do other centres use for pediatric iron deficient patients unresponsive to oral iron?
