medication reviews

Just wanting to put out a question for everyone working in a hemodialysis unit. How are med reviews done in your program and how often? Do nurses and pharmacists both do them? How are patients prompted to bring in their own medications? We try and see our hemo patients at least once a year for a full, comprehensive med review... we track this using our own Excel spreadsheet. The nurses are supposed to be doing med reviews every 3 months but we are finding there isn't a systematic way they are being done. We would like to hear about your experiences. This would be a good survey for the RPN newsletter :)
