patient requests high hemoglobin despite new guidlines

Some of our patients insist on having a high hemoglobin of 125-130g/L or more while on epo therapy. The new guidelines are quite explicit in keeping within the target of 110-120g/L and not going above.
Wanting feedback as to what other units are doing to accomodate patients who feel better at a higher hemoglobin and insist on these levels despite counseling about the potential for cardiovascular harm.
