DUTIES AND RESPONSIBILITIES:
To provide direct patient care in the management of drug therapy to achieve positive patient health outcomes, primarily for nephrology and/or renal transplant patients of the Diabetes Comprehensive Care Program, and coverage to other areas as needed. Direct patient care focuses on review of medication orders; selection and assessment of patients requiring pharmaceutical care; identifying, resolving and preventing drug related problems for a subset of patients; patient education; patient continuity of care; and the provision of drug information.
Other responsibilities include support of i)distribution of drugs throughout the hospital, including weekend, evening and on call rotations ii) educational and research programs iii) promotion of best practice in drug therapy and pharmacy practice.
QUALIFICATIONS
Legally eligible to work in Canada.
Registered as a pharmacist with the Ontario College of Pharmacists.
Hospital Pharmacy Practice Residency or equivalent.
Minimum of 1 year relevant clinical experience in providing pharmaceutical care to patients in a teaching hospital setting, with medical-surgical or nephrology experience preferred.
Excellent communication, interpersonal, organization and problem solving skills and demonstrated ability to function in a team environment.
Demonstrated ability for scholarly activity, patient and student teaching.
