ritiuximab (Rituxan) question

We have a PD patient with Wegener's who is not responding to the standard therapy of steroids and immunosuppressives. The nephrologist was wondering about using rituximab IV 375mg/m2 weekly x 4 weeks. This dose appears to be supported in the literature but I am more concerned about cost and logistics. The particular patient in question has private insurance which we are looking into. Does anyone have any practical experience with this drug? If so, how did you obtain the drug? Who paid for it? Where and how was it given? Much thanks in advance.
