A review of the trial that began March 2003 at Vancouver General Hospital studying the switch from SC Erythropoetin to IV Darbepoetin. Study objectives were 1) to assess the ability of Nesp (Aranesp) to maintain target Hemoglobins; 2) determine an appropriate dose conversion from Epo (Eprex) to Nesp (Aranesp); and 3) to perform a cost analysis. Study followed patients 6 months prior to switch and evaluated at 12 months post switch.

Dr. Bailie gave a brief history of the empiric treatment of peritonitis in ambulatory peritoneal dialysis patients. He included a discussion of the risks and cautions of resistance, pharmacokinetics, efficacy and current dosing recommendations for cefazolin, tobramycin, piperacillin and vancomycin. This was a very good overview which will benefit all renal staff involved in treating infections in their CAPD patients.

Dr. Mendelssohn reviewed the literature on the use of COX-2 inhibitors and patients with renal diesease. He reviewed the current recommendations for blood pressure control and the challenges to reaching these targets. Since the use of COX-2's might affect blood pressure, these were reviewed with respect to blood pressure control and other cardiac outcomes.

"Hyperhomocysteinemia: The Folic Acid Controversy"

This presentation was a review of the latest literature on Homocysteine in Renal failure and the role of Folic Acid in improving Cardiovascular outcomes. In essence, it was noted that 1mg per day (found in Replavite) is no better than 5mg except in the Prerenal (PRI) or transplant population, and even that data suggests that 1mg will lower homocysteine levels to almost normal levels. Current studies have looked at homocysteine and its correlation with improvment in endothelial wall function. Also included are references the end of the presentation for those would like to get more information about this topic. She welcomes any discussion about the information found in these slides or questions about the topic in general.
